Donna Mueller has more than 30 years in healthcare as a nurse, physical therapist, consultant and program specialist in post-acute care. Her leadership has established highly successful networks delivering outcome-based post-acute care models in response to value-based care initiatives.
Donna has assisted more than 100 post-acute facilities to secure appropriate ancillary services, develop niche clinical programs, evaluate emerging technology, and integrate evidence-based care. Donna helped develop and implement care re-design that led the Infinity PLUS network successfully into Bundled Payment for Care Improvement for the Avamere Family of Companies.
Donna earned her degree in physical therapy at the University of Washington. She is a lifelong learner and has completed extensive continuing education coursework in all areas of clinical practice and leadership, including an extensive executive training curriculum by Yale School of Management.
Sonia Rai is a highly experienced and creative leader who has successfully built two companies, her first in technology then healthcare. Her passion is new business development, mentoring leaders and teams, growing revenue streams quickly and consistently, and defining superior customer engagement.
Since taking the helm of Signature’s Home Health Portland office in 2010, she has driven all aspects of agency growth, tripling revenue during her tenure, improving quality ratings to a consistent 4-4.5 stars, and becoming the preferred home health provider for key acute and post-acute providers.
One of her passions is piloting leading-edge programs, including risk assessment, bundled payments, comprehensive joint replacement, patient management, and warm hand-off processes. Her efforts reduced agency re-hospitalization rates significantly, making her a valued player in the BPCI revenue share community. Sonia was honored to receive the 2018 Hope Runnels Award for outstanding leadership from the Oregon Association for Home Care.
Sonia graduated from Sheffield Hallam University in the UK with honors and a BSc in computer systems, systems analysis and methodologies.
